Tesamorelin

A Growth-Hormone-Releasing Peptide Therapy

What Is Tesamorelin?

Tesamorelin is a synthetic peptide that belongs to a class of compounds called growth hormone-releasing hormone (GHRH) analogs. Rather than supplying growth hormone (GH) directly, tesamorelin works upstream — it binds to GHRH receptors in the pituitary gland and stimulates the body’s own production and release of natural growth hormone. This, in turn, increases circulating insulin-like growth factor 1 (IGF-1), the hormone largely responsible for GH’s downstream effects on metabolism and tissue growth.

Tesamorelin is FDA-approved (brand names Egrifta®, Egrifta SV®, and Egrifta WR®) for the reduction of excess abdominal fat in adults with HIV-associated lipodystrophy. Because of its effects on visceral fat, lean body mass, and metabolic markers, it is also used off-label by physicians in medically supervised body-composition and healthy-aging programs. It is administered as a once-daily subcutaneous injection.

Potential Benefits

Clinical studies (primarily in HIV-associated lipodystrophy) and physician-supervised off-label use have associated tesamorelin with the following effects. Individual results vary, and benefits depend on appropriate patient selection and consistent use:

  • Reduction of visceral adipose tissue (deep abdominal fat around the organs), the fat most strongly linked to metabolic and cardiovascular risk
  • Improvement in waist circumference and abdominal body composition
  • Increase in endogenous (the body’s own) growth hormone and IGF-1 levels, rather than introducing exogenous HGH
  • Favorable changes in triglycerides and other lipid markers in some patients
  • Support for lean muscle mass preservation during fat-loss programs
  • Improvements in overall body composition when paired with nutrition, resistance training, and a comprehensive weight-management plan

Tesamorelin has a weight-neutral effect in its approved indication — meaning it is not designed to produce dramatic scale-weight loss on its own. Its primary, best-documented value is in reshaping body composition (less visceral fat, preserved lean mass) rather than as a standalone weight-loss drug.

Dosing Guidelines

Dosing must always be individualized and directed by your prescribing physician. The information below reflects standard FDA-labeled dosing for reference and patient education only — it is not a substitute for your provider’s specific instructions.

ParameterStandard Guidance
RouteSubcutaneous (SC) injection, typically into the abdomen
FrequencyOnce daily, at approximately the same time each day
Dosing Recommendation33 units (0.33 ml) daily
Injection site rotationRotate sites within the abdomen to reduce irritation and lipoatrophy risk
ReconstitutionOnly the diluent supplied with the product should be used; follow pharmacy/product-specific mixing instructions exactly
Missed doseDo not double up — resume the regular schedule at the next planned dose unless your provider advises otherwise

Before Starting Therapy

  • A baseline history and physical exam with your provider
  • Baseline IGF-1 level and relevant metabolic labs
  • Screening for contraindications, including active malignancy, pituitary or hypothalamic disease, and pregnancy

While on Therapy

  • Periodic follow-up labs (including IGF-1) to confirm the dose remains appropriate and safe
  • Monitoring for fluid retention, joint or muscle discomfort, and injection-site reactions — the most commonly reported side effects
  • Ongoing communication with your care team about how you’re responding and any new symptoms
